Tell Your Heart Story

Heart Stories Used to Make a Difference
We need your story accompanied by two photos. Family photos, individual photos of the person(s) with Congenital Heart Defects or hospital photos are all great examples. These stories will be used for important initiatives which will help carry out the mission to advocate for each and every one of you. Stories should highlight the overwhelming need for Congenital Heart Defects Research, a National Patient Registry, Congenital Heart Defects Education and Awareness.

Story Guidelines
There is no age limit and we want to hear from you whether the person with the Congenital Heart Defect is alive or deceased. It’s just as important to hear from grandma, grandpa, aunt, uncle, cousin, sibling, friend or whomever would be willing to write. Please highlight experiences that have been challenging and how this has affected your life. Many do not understand the seriousness, lifelong affect and financial devastation a Congenital Heart Defect creates on everyone involved. It is also important to include any other medical issues, learning disabilities and delays and emotional/mental toll it takes on those involved. Please understand that stories may be edited and may or may not be used for Lobby Day due to limitation of space/finances. Please help us to tell your story.

There are several ways to submit your story:

NOMINATE A NURSE

“Day after day thousands of heart failure nurses attend to the needs of the millions of Americans with heart failure largely without recognition,” said Dr. Tony Joseph, President of the Colloquium. “This award recognizes the competency and skills of heart failure nurses by affirming their important role on the lives of those with heart failure.”

Do you know a nurse that has made a significant impact on your life? Nominate them for the Heart Failure Nurse Recognition Award and honor their commitment to providing excellent heart care.
